Common Peroneal

Indications

  • Lacerations or abscesses involving lateral lower leg

Contraindications

  • Infection overlying injection site
  • Allergy to local anesthetic

Equipment

  • 5cc of local anesthetic of choice
  • 22-25G needle
  • Cleansing solution
  • Ultrasound with high-frequency linear transducer
  • Ultrasound transducer sterile cover

Preparation

Position

Position the patient supine

Ultrasound

Landmark

  1. Place the transducer in a transverse orientation inferior to the fibular head on the lateral aspect of the proximal lower leg

Demonstration of the anatomy of the common peroneal nerve at the level of the proximal fibula. This image was obtained with the linear transducer placed in a transverse orientation on the lateral aspect of the fibular head. The nerve (*) is seen just superficial to the hyperechoic fibular cortex.

  1. Identify the common peroneal nerve just superficial to the fibular cortex

Technique

  1. Introduce needle from medial aspect
  2. In-plane needle visualization
  3. Infiltrate local anesthetic superficial to the common peroneal nerve (*)
